Use the AEPA Score Report Request Form to request an additional copy of your score report for tests custom-developed for use in Arizona. The fee for this service is $15 per copy.

NES (National Evaluation Series) Tests

Use the NES Score Report Request Form to request an additional copy of your score report for a NES test. The fee for this service is $40 per recipient.

Foundations of Reading Test
